Purpose of the form: Pursuant to Law No. 119/2014 “On the right to information”, every person has the right to access public information, without being obliged to provide explanations on the reasons. He/she completes his/her request in writing according to the form and can submit it in person, by post or via e-mail, correctly presenting the identity of the applicant and his/her signature.
Mandatory accompanying document: Photocopy of the applicant's identity card.
